Low-cost drugs have a $30 copay while mail-order will reduce your costs with a $75 copay. X-rays are subject to a $125 copay and the deductible.
The deductible is $7,700, which is common with Bronze-tier options. The maximum out-of-pocket expenses are $8,550 with 50% coinsurance. Urgent-Care visits are also subject to coinsurance and a deductible, although mental/behavioral and substance abuse outpatient services must only meet a $60 copay. Prenatal and postnatal visits are covered. The CareSource provider network provides Hoosier residents with a large choice of doctors, specialists, and hospitals.
CareSource Marketplace HSA Eligible Bronze – HSA-eligible option with $5,300 deductible and $6,750 maximum out-of-pocket expenses. The coinsurance is 50%. A Health Savings Account is ideal for healthy households with no major medical conditions. Contributions do not have to be made to the savings portion to keep the policy active. Accumulated funds can also help pay for dental and vision costs, and some Medicare-related expenses. Once you reach age 65, deposits are no longer permitted, although accumulated funds may be used for qualified expenses.
Ambetter Essential Care 2 HSA – HSA-eligible option with $6,550 deductible and $6,550 maximum out-of-pocket expenses and 0% coinsurance. A Health Savings Account is ideal for healthy households with no major medical conditions. Similar to prior plan, but slightly less expensive. Network extends to additional states. Typically, this plan is more expensive than the HSA plan above.
Ambetter Balanced Care 12 – Office visit copays for primary-care physicians and specialists are $35 and $70. A deductible and coinsurance do not apply and there is no limit on the number of covered visits. The policy deductible is $6,500 with maximum out-of-pocket expenses of $8,150. $35 copay for lab outpatient and professional services. 40% coinsurance for diagnostic imaging and x-rays. Generic drug copays are $25 and $62.50 (mail order). The preferred brand drug copays are $60 and $150 (mail order). Non-preferred brand and specialty drugs are subject to the policy deductible.
UnitedHealthOne Short-Term Medical Value Select – The least expensive type of coverage available. Since this plan is designed to be kept less than 12 months, the rate is much lower compared to other unsubsidized options. However, coverage can be retained for up to 364 days.
This specific plan is a catastrophic major-medical contract that provides up to $2 million of benefits. A prescription drug discount card is provided, with an optional $12,500 supplemental accident rider. Deductible options are $2,500, $5,000, $7,500, $10,000 and $12,500. The coinsurance is 30% on all policies. The Urgent Care copay is $75 but pcp and specialist office visits must meet the coinsurance and deductible.
A discount prescription drug discount card is provided (inpatient drugs only). Drugs prescribed outside of a hospital are not included in benefits. ER fees for illness and injury are covered but subject to a deductible.
Golden Rule, a subsidiary of UnitedHealthcare, underwrites the plan. Although there are only a few medical questions, and approval can take less than 24 hours, applicants can be declined if they have been treated for cancer, heart disease, diabetes, or several other conditions.
